Proof of Identity in Person or by Mail

In person

The applicant must go to the consulate with:

-Laminated identity card (valid or not) or passport (with three copies of identity document submitted)

- Documents where it  can be verified your name and place of residence.

The document will be delivered the same day during business hours to the public.

By mail

The applicant must download  the form Proof of Identity from our website, complete the form in front of the notary (a) who will authenticate the form. Finally, the interested may apostille the document in the Secretary of State of his or her jurisdiction.
